I have placed Costco's return to compliance response to each issue in <br />parenthesis: <br />#3:Current CUPA Facility & Tank Forms (attached forms). <br />#4:Current Fin. Resp. Forms (attached forms). <br />#S:Monitoring & Response Plans not current (attached forms). <br />UDC 5/6; 7/8 & 9/10 sensors were not operating as per the monitoring plan. 5/6 & 7/8 <br />were reprogrammed prior to the completion of the inspection. 7/8 was replaced by Elite <br />IV on 8/18/09. (Elite IV work order #22398 attached). <br />#23: Designated Operator Notification form was not sent to Agency prior to DO <br />9/25/2009 <br />
