Advanced <br />GeoEnvironmental, Inc. <br />30 November 1998 <br />AGE -NC Project No. 95-0197 <br />` San Joaquin County Public Health Services - Environmental Health Division <br />304 East Weber Avenue, Third Floor <br />Stockton, CA 95202 <br />Subject: Tank Removal Application Addendum <br />r. <br />Paige's Towing - 1807 Douglas Road, Stockton, California <br />To Whomever Concerns: <br />An application to close two underground storage tanks (USTs) for the above referenced sire has been <br />submitted to the San Joaquin County Public Health Services -Environmental Health Division (PHS-EID) <br />for review. This addendum to the application is being submitted to address part 2 of the General Instructions <br />section of the application. <br />1) Prior to any excavation activities, Underground Service Alert (USA) Hill be notified a minimum of <br />48 hour for inspection of the above referenced site for underground utilities. <br />2) Both petroleum impacted soil and overburden soil generated during excavation activities will be <br />stockpiled on-site and covered with plastic sheeting in an area lacking access to the general public <br />(See attached Site Plan). Disposal alternatives will be evaluated based on results of soil analysis. <br />3) If any impacted soil is to be hauled off-site, than Jim Thorpe Oil, Inc. (JTO) has been selected as the <br />contaminated soil hazardous waste hauler. All correspondence should be mailed to P.O. Box 357, <br />Lodi, California 95241-0357.
