The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). According to the service requestor, no <br /> piping modifications will take place during the proposed conversion frames installation. <br /> a. Verification of conversion frames installation per the manufacturer's installation instructions <br /> b. Verification of the dispenser footprints contained by the under dispenser containment sumps <br /> (UDCs) and verification that all potential product piping leaks will be captured in the UDC. <br /> c. Visual inspection of the piping (in operating condition) within all dispensers for any product <br /> leaks <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of <br /> an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the Contractors State <br /> License Board (may be issued to the installation company), or a Tank Tester's license issued <br /> to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the International Code <br /> Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> tested, repaired, or serviced. <br /> 1868 E.
