The designated operator (D.O.) failed to attach alarm histories on the August, July, June, May, <br /> April, and January 2013, and December and October 2012 designated operator monthly inspection <br /> reports. The D.O. listed the alarms in the monthly reports, but failed to check that the alarms were <br /> responded to appropriately or attach documentation taken in response to the alarms. The alarms <br /> included the L2: 87 STP sump sensor fuel' alarms, L4: 91 STP sump sensor 'fuel' alarms and 'sensor <br /> out' alarms, L6: diesel STP sump sensor sensor out' alarms, and 87 PLLD gross line fail alarms. During <br /> the monthly inspection, the designated operator shall review the alarm history for the previous month, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> 305. The Bravo box float and chain in the dispenser 5/6 (center) sump and dispenser 7/8 (north) failed <br /> to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained <br /> to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br /> leak. The service technician adjusted the floats and chains and verified functionality. <br /> REPEAT VIOLATION <br /> 313. The diesel direct-bury spill container failed when tested. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br /> replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the spill container can't be replaced immediately, there is a possibility that the diesel UST system may <br /> be red tagged to prevent fuel inputs. <br /> REPEAT VIOLATION <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector: <br /> Rocei Y: Tits: <br /> EHD 2302 04119 2 Page 3 UST INSPECTION REPORT <br />
