Charter Way, Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> ROUTINE ANNUAL INSPECTION AND MONITORING SYSTEM CERTIFICATION <br /> REVISED REPORT 6/16/11 - This report supercedes the previous, May 31, 2011 inspection report. <br /> "The annual paperwork inspection was conducted on May 31, 2011. The monitoring system <br /> certification was conducted on May 31, June 13, and June 16, 2011. <br /> Violations: (6, 7, 23c, 24, 25) <br /> 6, 7, 23c. A copy of the current approved monitoring and response plans were not found on site. A copy <br /> of the UST operating permit was not found on site. Maintenance and monitoring records were not found <br /> on site. The designated operator monthly inspection reports were not found on site. According to the <br /> owner, the binder containing all of the paperwork for this facility was stolen from the store while it was <br /> under construction. These records shall be kept on site for at least 3 years. The monitoring and <br /> response plans must be current and approved by the EHD with a copy maintained on site at the facility. <br /> A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Immediately obtain a copy of the permit and retain it on site. Ensure that all of these documents are <br /> maintained securely on site at all times in the future. <br /> 24 & 25. Annual monitoring system certification, leak detector testing, and spill container testing were <br /> last performed on May 26, 2010 and was completed on June 16, 2011, one month past due. These <br /> tests are required once every 12 months. Take all necessary precautions to ensure testing is performed <br /> in a timely manner. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by June 30, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins dor.
