SUBMITTAL OF PLANS <br /> F <br /> � <br /> a. Submit 3 completed copies of the permit application along with a "PHS-EHD Service <br /> Request" form to the above letterhead address. <br /> b. A permit will be issued by PHS-EHD upon review and approval of the application and <br /> pians. The required fees must be submitted with the application package. Information <br /> in addition to that presented in the application may be needed to obtain final approval. <br /> No work is to begin on the proposed project until a permit has been issued. The <br /> required inspections cannot be scheduled until a permit is issued. <br /> C. Prior to tank lining, a site assessment must be performed under the direction of PHS- <br /> EHD, Site Mitigation Unit. Please contact the Site Mitigation Unit at (209) 488-3450 for <br /> further information regarding the assessment. <br /> d. If the UST facility wishes to comply with the 1998 upgrade requirements in conjunction <br /> with the tank lining, then equipment and material specification sheets and <br /> manufacturer's brochures must be submitted for all proposed equipment. All proposed <br /> leak detection methods and equipment must meet the requirements specified in 40 <br /> CFR 280.40 and have been reviewed by the State Water Resources Control Board. If <br /> the UST facility does not meet the upgrade requirements by December 22, 1998, then <br /> the tank(s) must be closed according to Article 7, Califomia Code of Regulation (CCR). <br /> C. After permit approval, it is the responsibility of the applicant to notify PHS-EHD at least <br /> 48 hours in advance to schedule the required inspections. <br /> 2. PLAN REVIEW AND PERMIT FEES <br /> a. UST lining or repair plan review fee is $234.00 per tank. Upgrade plan review fee is <br /> $234 per facility. Additional time spent on plan review, meetings, inspections, phone <br /> or office consultations will be billed to the applicant at a rate of 878.00 per hour. <br /> b. The maximum review time for UST interior lining or repair plans is 40 working days. A <br /> preliminary review will be conducted by PHS-EHD within 20 working days of submittal. <br /> C. If gross deficiencies are identified, the plan will be returned with a checklist identifying <br /> plan deficiencies. <br /> 3. PLAN REVIEW GUIDELINES <br /> a. The attached plan review sheet is to be completed and submitted to PHS-EHD. <br /> b. Permits and/or plan check approval may also be required by the San Joaquin Vailey <br /> Unified Air Pollution Control District, the County/City Building Department and the Fire <br /> District in your respective area. <br /> C.
