1. Prior to tank lining, a site assessment must be performed under the direction of Public Health Services <br /> Environmental Health Division (PNS-EHD), site mitigation unit. <br /> J Site Assessment has been performed under PHS-EHD direction <br /> Site Assessment has not been performed under PHS-EHO direction. Reason tanks will be interior lined/repaired: <br /> Meet State/Federal upgrade requirements (submit PHS-EHD UST upgrade permit application <br /> with this application) <br /> Tank system failure, please describe: <br /> 3. The following tests are required to be performed prior to lining to determine the structural integrity of the <br /> n } UST: <br /> Steel tanks <br /> Ultrasound Test (Note: The UST shall be closed in accordance with Article 7 Title 23 CCR if the <br /> tank's average metal thickness is less than 75 percent of the original 'mall thickness of if the tank <br /> has any of the listed defects in Article 6, Title 23 CCR section 2663 (b) (2) (B)) <br /> Certification by a "Special Inspector' that the tank is structurally sound for tank lining and <br /> continued use: <br /> a_ sandblast to white metal <br /> �b. count number of holes, splits, perforations, and other defects <br /> ----c. submit result to EHD <br /> Non-steel tanks, fiberglass <br /> Certification by a "Special Inspector" that the tank is suitable for lining. <br /> _a. interior walls cleaned by wipe down <br /> b. interior diameter measurements <br /> _c. visual check for structural, tension, or compression problems <br /> 4. Corrosion protection system <br /> 1 ' <br /> rule Please describe the type of corrosion protection system you propose to install. The cathodic protection <br /> system retrofitted on existing UST systems must be designed by a corrasion specialist certined by the <br /> National Association of Corrosion Engineers (MACE) or by a qualified professional engineer. Metal <br /> components of the UST system (i.e. submersible pumps, piping etc) that rcutineiy contain fuel and are in <br /> contact with the ground mus; also be cathcdically protec,ed. <br />
