Box 508 <br />Stockton, CA 95201 SENT VIA FAX <br />Re: Ryder Truck, 3633 Duck Creek Dr., Stockton CA <br />The permit (SR0053257) to repair the waste oil secondary containment sump for the underground <br />storage tank system (UST) at the above referenced site has been approved with the following <br />conditions. <br />Conditions of approval: <br />1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits. <br />?. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supersede <br />the previous ones. <br />3. All components must be certified by an independent testing organization. (UL, ULC, <br />ANSI, API, ASTM, etc. See California Code of Regulations (CCR), Title 23 Chapter 16 <br />Table B for complete list). The component must bear the appropriate mark as required by <br />the independent testing organization. <br />4. If concrete is broken near the UST, a tank integrity test and/or line integrity test (or <br />equivalent) must be performed, once concrete is poured in accordance with the <br />manufacturer's guidelines. Results must be received by EHD within seven days of <br />returning the facility to operating condition. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />
