%.. 1.04 <br /> SITE HEALTH & SAFETY PLAN <br /> DIESEL STORAGE TANK FILL-IN-PLACE <br /> 2749 S. ELDORADO STREET <br /> STOCKTON, CALIFORNIA <br /> February 8, 1998 <br /> 1. FACIIdTY BACKGROUND/WORKPLAN <br /> Project consists of filling in place of one underground storage tank. Said tank used to be a diesel(new <br /> product)holding tank for disbursement to vehicles. Testimony indicates that it has been in disuse for <br /> many years. Soil sampling beneath the tank was performed on December 1, 1997 and subsequent testing <br /> indicated that no traces of hydrocarbons were detected. Presently,the tank has been cleaned rendered <br /> inert by rinsing. Rinsing and rendering inert was done by Clearwater Environmental Services on <br /> February 3, 1998. Subsequently,on the same date,two holes were cut in the top of the tank to facilitate <br /> filling of same. The holes are near each end and are approximately 1 foot square in size. <br /> Work tasks include the following: <br /> Backfilling the tank with a weak concrete slurry mix consisting of one bag of cement per cubic yard of <br /> concrete(the cement is intended to act as a lubricant,helping the concrete to slump throughout the tank); <br /> Backfilling the two access excavation areas with native soil(approximately 3 cubic yards of backfill); <br /> Securing the site in a safe manner; <br /> 2. KEY PERSONNEL and RESPONSIBILMES <br /> Key personnel will consist of the following people: <br /> Concrete truck operator employed by the chosen concrete plant. Ken Willis,owner and operator of heavy <br /> construction equipment. Mr. Willis,along with the above operator,will guide the concrete chute to <br /> effectuate the filling of the tank. Subsequently,with the help of his construction equipment Mr.Willis <br /> will backfill the 3 t cubic yards of native soil and smooth to match the existing ground. Mr. Willis'. <br /> address is 321 1"Avenue S.,Pacheco, CA. 510-687-6244. <br /> Ivan Vegvary,Registered Civil Engineer&Licensed Land Surveyor will observe the operations and <br /> provide any necessary reports required by County Health and/or County Fire departments. Mr.Vegvary <br /> might also assist in the above work scope,if needed Being that he is an independent contractor-sole <br /> proprietor professional, he need not be covered by workman's compensation insurance.
