I have reviewed those documents and have questions or <br /> comments on the following. <br /> 1. The City of Stockton Fite Department needs to approve the underground storage tank closure in place before <br /> Environmental Health can approve it. Please provide that approval. <br /> 2. Mr. Willis indicated the tank and/or piping may be on neighboring property,before any permits can be issued the property <br /> must be surveyed to show the property line. If the tank and/or piping is on neighboring property you must provide approval <br /> from the owner of the neighboring property indicating they agree to a deed restriction on their property title. Also,no <br /> borings permits can be issued without a Written Access Agreement from the neighboring property owner if borings will be <br /> drilled from or on neighboring property. It is hard to tell from the drawings submitted which direction the borings will be <br /> drilled. A boring permit is required for each property. Please explain in detail exactly how the borings will be obtained to <br /> prevent any possibility of drilling through the tank. <br /> 3. The check submitted was in the amount of 5274 instead of 5234. Would you like us to return the check and have you <br /> reissue it? Or would you prefer to apply it to any additional costs,if any? <br /> 4. The Closure application is still lacking an approved contractor. <br /> 5. Hazardous waste will be generated because the tank is required to be decontaminated by rinsing unless the tank is to be <br /> removed and hauled as hazardous waste. This requires an EPA ID number. After the tank has been decontaminated by a <br /> qualified contractor it will then need to be inerted. Please provide that information in the application. <br /> 6. The underground storage tank regulations require the piping to be removed which will also require the piping to be <br /> decontaminated unless the piping is to be hauled as hazardous waste. <br /> 7. The piping will need to be sampled every twenty feet. <br /> 8. Describe the inert solid that will be used to fill the tank. <br /> 9. Submit a Site Safety Plan explaining in detail all potential hazards at the site including grounding and bonding of the tank. <br /> 10. The final samples beneath the tank need to be taken in native soil which may be beyond two feet below the tank. <br /> 11. The Authorization to Release farm needs to authorize a laboratory,it was left blank. <br /> The items above are not listed in any order. The first step necessary is to obtain written permission for the closure in place from <br /> the fire department Once that step has been completed the boring permit(s)need to be obtained and the borings done under <br /> Environmental Health's supervision. Once those borings come back clean the closure in place can be approved if the above <br /> items have been submitted. The final comment is that we need a new service request form signed by Mr. Willis and submitted <br /> to this office.
