The permit has been approved with the following <br /> conditions: <br /> Note: This application was submitted after work had been completed at the site. <br /> Inspection is still required. <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> a. Contact Michelle Henry to arrange the following inspections (a <br /> 48 hour notice is required): Verification of monitoring system to <br /> ensure it was unaffected by the EVR phase II upgrade. If necessary a <br /> functionality test must be performed to verify monitoring system meets <br /> the requirements of California Code of Regulations Title 23 and the <br /> Health & Safety Code. <br /> b. A copy of the alarm history prior to commencement of work and <br /> after completion of work must be available for review at the time of <br /> EHD inspection. <br /> c. Secondary containment testing of all product lines and UST's, <br /> immediately. <br /> 2. Qualified installers must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site, at all times while <br /> work is being performed, who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. All valid, current California UST certificates issued by the International <br /> Code Council (ICC), that are applicable to the work being performed, and <br /> Page 1 of 2 <br />
