Ensure a copy of the "Site Health and Safety Plan" is available on the jobsite as required by Title 8. 3. Detailed description of work to be completed. List com ponents to be repaired or replaced and attach a diagram drawn to scale showing location of repairs and/or replacements. If repairing a component, describe how this will be done. (If adding piping, UDC's, or other UST equipment, or performing tank top upgrade, use the US T Installation Application pages 4-8 as necessary for a timely plan review): Install Phase II EVR upgrade per CARB Executive Order VR-202: Healy System with Incon In-Station Diagnostics. No new under dispenser penetrations will be installed within the scope of work of the Phase II EVR installation. Work includes exposing existing underground low voltage conduit in front of existing dispensers for connection of Healy TS-VFM flow meters to new Incon TS-550 console in equipment room(see attached plans for details). Cold Start of the existing Veeder Root TLS350 tank monitoring system is not required. 4.
