The permit <br /> has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Overfill prevention verification: Verify that external audio and visual alarm activates <br /> when tank is 90% full. <br /> b. Corrosion protection: Verify corrosion protection for 4" steel riser prior to backfilling. <br /> c. Secondary containment testing on 87 tank. <br /> Please be aware that the facility cannot operate receive fuel in the 87 tank prior to <br /> the EHD inspection and approval. <br /> 4. Since concrete will be broken near the 87 tank, a secondary containment test must be <br /> performed on the 87 tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
