Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on February 15, 2012, and a test report was not received within thirty days by the EHD. The <br /> test report was received on April 5, 2012, almost two months late. A copy of the test report must be <br /> submitted within 30 days of the tests. Take the necessary precautions that a copy of the test report is <br /> submitted to the EHD within thirty days of the test. <br /> 204. The designated operator failed to check and note the correct test dates for the spill container test <br /> on the August 2012 through January 2013 designated operator monthly inspection reports. These <br /> reports listed the spill container test as July 5, 2012. It should have been listed as July 11, 2012. During <br /> the monthly inspection, the designated operator shall check that all required testing and maintenance for <br /> the UST system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> 303. The 91 turbine sump sensor was raised approximately three inches off the lowest point of the sump <br /> part way up the saddle of the sump bottom and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> During the inspection, the technician relocated this sensor to the lowest point of the 91 turbine sump. <br /> Continue to ensure that the monitoring system sensors are properly located to detect a leak at the <br /> earliest opportunity. <br /> 305. The 87 and 91 Bravo box floats and chains in the UDC#3/4 & 5/6 sumps failed to stop the flow of <br /> product at the dispenser when tested. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service <br /> technician adjusted the chains and verified functionality. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> Inspector ..
