Eleventh Street, Tracy <br />The proposed application for permit to remove one underground storage tank at the above - <br />referenced location, has been approved with the following conditions: <br />1. All inspections must be scheduled at least 48 hours in advance. <br />2. Call USA Dig prior to any excavation. <br />3. Underground Storage Tank Disposition Tracking Record must be submitted within 30 days <br />of tank removal. <br />4. Verification of backfill material must be submitted within 30 days of the tank removal. <br />5. The Tank Closure Certification form must be submitted within 30 days of the tank removal. <br />6. Sample analysis must be conducted in accordance with the unknown fuel specifications on <br />page 8 of the permit application. This page was last updated on 5/9/00. Copy attached. <br />7. The amount of samples taken will be based on the specifications on page 7 of the permit <br />application. <br />8. Analytical results must be submitted directly from the lab to PHS-EHD within 30 days of the <br />tank removal. <br />9. Per phone conversations with Richard Thorpe, a grading and excavation permit has been <br />obtained from the City of Tracy (01-258), and the hazardous waste hauler is American <br />Valley (hauler #3744). <br />10.
