Facility ID: SKTNCA14 <br />Facility Name: Pacific Bell <br />Emergency Response Plan <br />3) National Response Center 800-424-8802 <br />4) Pacific Bell Emergency Control Center 510-823-7777 <br />5) Environmental Management 510-823-9824 <br />6) Real Estate Department 510-823-1031 <br />7) Respective upper management within group. <br />In case of fires, the local Fire Department and Pacific Bell Emergency Control Center will be notified <br />promptly by the operator of the underground tank. <br />In each step of notification, good documentation will be prepared to include: <br />1) Location <br />2) Specific description and location of the area involved <br />3) Time and date of occurrence <br />4) Description of spill and type of material <br />5) Approximate quantity of material that spilled/leaked <br />All recordable leak and spill records will be maintained on the site. <br />The key personnel on the site (Building Warden and Site Manager) are trained for responding <br />appropriately to any major catastrophe. <br />A training program conducted by the training department of Pacific Bell provides the information to the <br />key personnel regarding emergency telephone numbers, reporting requirements, location of emergency <br />equipment and evacuation procedures. <br />This information is true and correct. If there is any change which would materially affect the Hazardous <br />Material Management Plan, we will amend the appropriate sections. <br />
