Financial responsibility documents have not been submitted to the EHD for 2011. The most recent <br /> financial responsibility documents submitted are dated 5-1-10. Current financial responsibility documents <br /> are required to be submitted annually. Complete and submit a copy of the financial responsibility by <br /> 6-19-1#. Repeat Violation. <br /> 23c. The designated operator failed to check and note the correct test dates for the monitoring system <br /> certification testing and spill container testing on the designated operator monthly inspection reports <br /> beginning with June 2010 through March 2011. D.U. had marked the most recent test date for these <br /> tests as 6-11-10. It should have been marked as 5-11-10. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports.
