Hwy 88 Lockeford, CA 95237 SR0061732 <br /> The San Joaquin County Environmental Health Department(EHD)has completed the review of <br /> the application for a permit to install an OPW 71 SO drop tube on the 87 underground storage <br /> tank (UST) system at the above noted address. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at(209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume <br /> shut off <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test(or equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the 87 tank prior to the EHD <br /> inspection and approval. <br /> 4.
