The designated operator employee training for Muhammad Luqman(who was working alone at this <br /> facility) was not performed for this facility. The list of employees trained by the designated operator for <br /> this facility do not include Muhammad Luqman. The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every <br /> 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by June 13, 2013. <br /> 303. The diesel turbine sump sensor was touching the bottom of the sump but was located <br /> approximately seven inches away from the lowest point of the sump, part way up the sump saddle, and <br /> not located to detect a leak at the earliest opportunity. The 89 turbine sump sensor was touching the <br /> bottom of the sump but was located approximately five inches away from the lowest point of the sump, <br /> part way up the sump saddle, and not located to detect a leak at the earliest opportunity. Monitoring <br /> equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. During the <br /> inspection, the technician repositioned the sensors to the lowest point of the sumps to detect a leak at <br /> the earliest opportunity. Take the necessary precautions to ensure that all sensors are located at the <br /> lowest point to detect a leak at the earliest opportunity. <br /> 305. The diesel Bravo float and chain in the diesel south satellite dispenser sump failed to stop the flow <br /> of product at the dispenser when tested. The chain lacked a clip to attach the chain to the Bravo float. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak.
