See pages 7 & 8 of the permit application for required analytical tests. <br />2. A copy of the fire department permit must be submitted at the time of removal. <br />3. Schedule removal appointment 48 hours in advance. <br />4. In addition, the following information must be submitted within 30 days of the removal <br />date: <br />(a) Analytical results, mailed from the lab <br />(b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br />(c) Tank Closure Certification Form <br />(d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt if <br />applicable <br />(e) Submit the backfill excavation certificate as required by San Joaquin County <br />5.
