French Camp Road,Manteca <br /> The proposed application for permit to remove two underground storage tanks at the <br /> above referenced location,has been approved with the following conditions: <br /> 1. All inspections must be scheduled at least 48 hours in advance. <br /> 2. Call USA Dig prior to any excavation. <br /> 3. Underground Storage Tank Disposition Tracking Record must be submitted <br /> within 30 days of tank removal. <br /> 4. The Backfill Excavation Certificate must be submitted within 30 days of the tank <br /> removal with verification of backfill material. <br /> 5. The Tank Closure Certification form must be submitted within 30 days of tank <br /> removal. <br /> 6. Sample analysis must be conducted in accordance with the unknown fuel <br /> specifications on page 8 of the permit application. <br /> 7. Analytical results must be submitted directly from the lab to PHS-EHD within 30 <br /> days of the tank removal. <br /> 8. Any remediation of contaminated soil is not covered by this permit.
