A site plan must be submitted identifying the locations where monitoring will be performed. Immediately
log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.govi and upload a copy of
the UST Monitoring Site Plan.
Xonnta
Failed to submit secondary containment test report within 30 days.
nmenttesting was scheduled to be performed on 5-27-2015 and a test report has not been
HD. A copy of the test report must be submitted within 30 days of the test. Immediately submit a
port to the EHD or schedule this test and provide 48 hours notification to the EHD.
Violation#201 -Failed to maintain alarm logs and/or records of follow up actions.
Monitoring records (monitoring system certification and the secondary containment test reports)for the 2015 were
not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1)
date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3)
results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all
readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of
inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for
the last three years on site and submit copies to the EHD. Submit proof of correction to the EHD.
Violation#321 -Overfill prevention was overridden.
The mechanical overfill prevention device on the 87 UST was overridden by the presence of a plastic(flexible) hose
placed in the drop tube. The hose, which measured approximately 135 inches, was removed by the service
technician on site. Mr. Harprit Johal indicated that he didn't know about the hose being present inside the drop tube,
and stated that the previous operator was probably stealing gas via this hose. Mr. Johal also stated that about a
month ago they were checking the fuel level for the air quality testing (with a measuring stick)and he did notice that
the stick"would not go all the way through", he also stated that at that time he was not sure why it was happening
nor did he realize that there is anything unusual about it. His stated that he was planing on asking the service
technician about it today. Take all necessary precautions to prevent this in the future
LDCD
OVA
-,-P . w
 32 Do o✓ l.e \agz InDu r.`.0 .
