The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Vicki McCartney at (209) 468-9852 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Sensor functionality: Functionality and communication of the monitoring system must <br /> be verified to ensure that monitoring has been restored to meet the requirements of <br /> Title 23 California Code of Regulations and the Health & Safety Code. <br /> b. Functionality of the new dispensers by confirming that the under dispenser <br /> containment will collect all potential leaks and verify no leaking pipes. <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br />
