The permit has been <br /> approved with the following conditions. <br /> Conditions of approval: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus (209-468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. A soap test to verify the pipeline integrity, Isolate and pressurize the pipe <br /> following the manufacturer's guidelines. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council(ICC), and <br /> C. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired,or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site, at all times while work is being performed, <br /> who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid,current California UST certificates issued by the International Code <br /> Council(ICC), that are applicable to the work being performed,and <br /> Page 1 of 2 <br />
