The permit <br /> has been approved with the followinq conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda superseae the previous one- <br /> 3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br /> n�pointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Overfill prevention verification: Measurement of drop tube to verify 95% tank volume <br /> shut off. <br /> b. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> c. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the Diesel tank prior to the <br /> EHD inspection and approval. <br /> 4. Since concrete will be broken over the Diesel UST, a secondary containment test must <br /> be performed on the UST once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
