• Prepare a WC Accident/Incident Report and submit to HSO/HSC. <br /> The form can be found in Section 10.0. <br /> 6.7.4 Monitoring Guidelines <br /> Organic vapor and explosive gas monitoring will be conducted any time <br /> a team conducts intrusive activities. The SSO or assigned HST will be responsible for <br /> performing ambient air quality monitoring with an organic vapor detector and a combustible <br /> gas indicator approved by the HSO/HSC. Details concerning organic vapor monitors, <br /> combustible gas indicators and their calibration are given in Appendix C. <br /> Air monitoring will be performed continuously and recorded at 15 minute intervals during <br /> intrusive operations. All monitoring data must be entered on the project safety log forms. <br /> This information will be used to determine if personal protective equipment requirements <br /> should be modified. <br /> Because toxicity action levels are considerably lower than explosivity action levels, organic <br /> vapor monitoring will be conducted in the breathing zone of the workers closest to the source <br /> of contamination. The worker's breathing zone is defined as a 1 foot diameter sphere <br /> surrounding the worker's head. <br /> Explosivity will be monitored with the CGI set at a location close to and downwind of the <br /> source of emission. The alarm on the CGI will be set to sound at the action level. If the <br /> alarm sounds during monitoring, shut-down and evacuation procedures will be initiated <br /> immediately. <br /> 6.8 AREA CONTROL <br /> I <br /> Access to hazardous and potentially hazardous areas of spill sites will be controlled to reduce <br /> the probability of occurrence of physical injury and chemical exposure of field personnel, <br /> visitors, and the public.
