Most of them result from the <br /> annual testing. Please see attached.The DO will address all of these alarms moving forward. <br /> Violation 209—A DO changed occurred during the course of the file review (the year).The DO, David Tucker, <br /> understands his obligation as designated operator to review the alarms each month. <br /> Violation 211—Training documentation attached.
