Designated operator monthly <br /> inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the <br /> previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies of <br /> the missing report to the EHD. <br /> This is a minor violation. <br /> 211 CCR 2715(f)Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for employees was not current. The last employee trainings were <br /> conducted on 8/17/2016 and has not yet been renewed. The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. <br /> The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> On 12/272016 the designated operator arrived on site and found active alarms on L16, L18, and L34. The L16, L18, <br /> and L34 sensor was found to be in alarm since 12/9/2016, 12/15/2016 and 12/17/2016, respectively,and actions <br /> have not been taken to address the alarm. The underground storage tank system shall be operated and maintained <br /> to prevent unauthorized releases. Ensure the underground system tank system and all monitoring equipment is <br /> maintained and operated to prevent unauthorized releases. Immediately document and maintain the underground <br /> storage tank system and submit documents to EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report which replaces the initial checklist.Violation#301 has been added. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Note:A new designated operator, David Turner, performed the August 24, 2017 monthly inspection and <br /> notification has not yet been provided to the EHD.
