The missing alarms include fuel <br /> alarms that occurred on these sensors and dates: <br /> -L3: 9/29/2016 <br /> -L4: 9/7/2016, 9/8/2016, 9/21/2016, 9/29/2016, 11/13/2016, 11/11/2016, 11/16/2016 <br /> -1_16: 9/22/2016, 10/09/2016, 10/11/2016, 5/3/2017 <br /> -L17: 10/11/2016 <br /> -L18: 10/16/2016, 11/28/2016, 2/8/2017 <br /> -L21: 2/23/2017 <br /> -L23: 2/10/1017 <br /> -L34: 9/14/2016, 9/15/2016, 9/16/2016 <br /> -High Liquid Alarms on L33 on 5/29/2016, 4/4/2016, 3/24/2016, 3/20/2016, and 2/17/2016. <br /> -Low Liquid alarm on L33 on 4/4/2016 <br /> -S1 had a No vacuum alarm on 8/28/2016 and 9/5/2016 and a vacuum warning alarm on 9/16/2016 <br /> -S2 had a no vacuum alarm on 9/2/2016, 9/13/2016, 2/2/2017, 3/8/2017, a vacuum alarm on 2/13/2017, and a <br /> communication alarm on 3/26/2017 and 3/8/2017. <br /> -S3 had a vacuum warning on 9/13/2016. <br /> Designated operator did not print all sensors alarm histories printouts: <br /> -S (smart sensor)-were not included on August 2017 printout <br /> -Q (leak detector) and T(overfill/tank) -were not included on any printouts. <br /> Work orders and alarm response documents were not attached to the monthly designated operator report. <br /> Designated operator did not verify that alarms have been responded to appropriately. The following were <br /> documented in the designated operator monthly report but no record of response was documented or found on site. <br /> Fuel alarms for L16 on 5/22/2016 and L23 on 1/7/2017 and a high liquid alarm on L33 on 3/24/2017. <br /> The designated operator response to the L33 Bio/Def annular space alarm, on 2/23/2017, noted that he found water <br /> in the annular space causing "High Liquid Alarm." He stated that water was removed from the annular space and <br /> the alarm cleared. A brine filled annular space should never contain water. Ensure the designated operator is <br /> documenting accurate responses to alarms. <br /> During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done.
