11.05/99 04:45 V713 9-4 9800 MAJESTIC TECH Z014 <br />- All excavated soils (spoils) shall be placed a minimum <br />of two feet back from the edge of the excavation. <br />The use of heavy and/or vibrating equipment (with the <br />exception of the excavating equipment itself) ad�acent <br />to excavations shall be prohibited. <br />All excavated areas shall be clearly marked and/or <br />fenced to prevent unauthorized and/or accidental entry <br />into work areas. <br />Necessary foot and vehicle traffic shall be directed <br />away from and around excavation work areas, and the <br />routes clearly marked. <br />Excavations shall be protected from intrusion of water. <br />8.3.6 CONFINED SPACE ENTRY <br />The scope of work does not require personnel to enter into <br />tanks, excavations, or other confined spaces. Field <br />personnel are strictly forbidden to enter any confined <br />spaces, for any reason, as a safety mandate of this SHP. <br />Therefore, the SHP does not currently address confined space <br />entry hazards and/or safety procedures. The SHP shall be <br />revised, reviewed, and approved prior to any confined space <br />entry work, should this operation become necessary. <br />8.3.7 HOT WORK• <br />Hot work may be defined as any operation which may <br />potentially create a source of ignition. This operation is <br />extremely dangerous when conducted anywhere that a flammable <br />and/or explosive atmosphere (or fire hazard) may be present. <br />because the tanks and pipelines to be removed and scrapped <br />are expected to contain at least some residues (if not <br />measurable Quantities) of gasoline (i.e., flammable or <br />combustible, depending on the substance). sources of <br />ignition shall be eliminated during all operations that mAy <br />interact with tank atmosphere. <br />For tank removal Operations. all tanks Shall be inerted <br />using dry ice at 30 lbs. per 1.000 gallons. tank <br />atmospheres shall be checked using a properly Calibrated <br />combustible gas meter for percent oxygen (% Ox) prior to <br />removal from the ground. AND prior to opening and scrapping. <br />Representative mamples shall be taken from various <br />locations/levels within each tank to ensure that the <br />environment is consistant. Prior to lifting and cutting. <br />stank atmospheres must be less than 10 percent LEL (<log <br />LEL) and less than 12 percent cyygen (<12% Oma•). <br />
