You need to obtain an EPA I .D. #• <br /> 2 . Please submit a "Site Specific Health & Safety Plan. " <br /> 3 . The dry ice requirements for inserting the tanks are 30 <br /> lbs/1000 gallons for gasoline, and 15 lbs/1000 gallons <br /> for diesel fuel . <br /> 4 . The responsible party to be billed for additional PHS- <br /> EHD staff time if expended beyond the 3 hour minimum <br /> permit payment per tank needs to acknowledge this <br /> responsibility for the billing, by signature and date <br /> on page six of the closure application. <br /> S . The owner of the facility must sign and date the <br /> "Authorization To Release" on page nine of the closure <br /> application. <br /> 6 .
