GENERAL HEALTH AND SAFETY REQUIREMENTS <br /> A. Training <br /> All contractor and subcontractor field personnel assigned to this operation will have <br /> completed applicable Health and Safety Training (i.e., Respirator Training) within <br /> the last 12 months. <br /> B. Medical <br /> All contractor and subcontractor field personnel assigned to this operation will be <br /> requested to have completed a comprehensive medical examination within the last <br /> 12 months. <br /> C. Field Supervision <br /> All field activities will be supervised by a Field Supervisor having training and <br /> experience commensurate with the activity. Field Supervisors have the authority <br /> and responsibility to correct any unsafe act or condition during the course of <br /> operations. <br /> D. Safe Work Practices <br /> All contractor and subcontract personnel assigned to the operation will follow safe <br /> work practices including good personal hygiene, reporting all injuries, following <br /> instructions, inspecting equipment before use, anticipating hazards, utilizing the <br /> buddy system, etc. <br /> E. Job Safety Review <br /> Field Supervisors will conduct daily pre-operations safety meetings with all <br /> contractor and subcontractor personnel. <br /> VII. SPECIFIC HEALTH AND SAFETY REQUIREMENTS <br /> A. Site Control <br /> Work zones and other site control measures will be established as needed for the <br /> operation, depending on chemical hazards, work location and other requirements. <br /> 3 <br />
