Kettleman Ln., Lodi, CA 95240 <br /> The San Joaquin County Environmental Health Department (EHD)has completed the <br /> review of the application for permit to replace the diesel leak detector at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Toua Yang at(209) 468-3459 to arrange the following inspections <br /> appointments (a 48-hour notice is required for inspection appointment(s): <br /> a. Leak Detector Test: Leak detector must be tested to confirm that the <br /> device can detect a leak of 3 gallons per hour at 10 psi. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. All components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing organization. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired, or serviced <br /> and <br /> d. Each person installing or testing a component must hold a certificate of <br /> training from the manufacturer of that component. <br />
