The missing alarms include: L-16, L-13, L-7 occurred 12/31/2014 <br />L-7, L-4, L-3, L-2, L-1 occurred <br />3/5/2014 <br />L-14, L-4, L-3, L-2, L-1 <br />occurred 1/30/2014 <br />L-4, L-3, L-2, L-1 occurred <br />1/27/2014 <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that <br />each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. Also ensure the <br />designated operator is visually inspecting all spill containers, UDC sumps, and tank top sumps that had an alarm for <br />which there is no record of service visit. Repeat violation CLASS II <br />319 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the diesel, 87 master and 87 syphon fill sump . If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician <br />removed a total of-3gallons of liquid from the sumps. Ensure that all sumps and annular spaces are maintained free <br />of liquid. Repeat violation CLASS II <br />324 <br />CCR 2635(b) No spill bucket installed or failed to provide a means to keep it empty. <br />All spill containers, Diesel, 91, 87 master, and 87 syphon, were approximately 1/4-1/2 full of liquid. All spill containers <br />shall have a minimum capacity of 5 gallons and be capable of containing a spill or overfill until it is detected or cleaned <br />up. Immediately remove any liquid in the spill container and dispose of according to Title 22 hazardous waste disposal <br />requirements. The DO is responsible for inspecting spill containers, documenting and notifying operator if there is <br />liquid to be removed. Submit a statement how the facility will ensure spill containers will be maintained clean and free <br />of liquid at all times. <br />Note: Liquid removed from the spill container and placed in the hazardous waste drum by UST service technician. <br />Repeat violation. CLASS II <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by February 14, 2015, 30 days from now. <br />Note: Checklist of violations left onsite January 7, 2015. Complete written report brought to site Janaury 14, <br />2015. <br />Consent to perform the inspection, take photos and collect samples was given by Wendy Raney, Manager. This <br />facility's designated operator is Ryan Monte (ICC expiration date: 11/16 ) .
