(039-0507807) 12,000 gallon (aviation gas) <br /> from the above referenced facility has been approved with the following conditions: <br /> 1. If the backfill material and soil is found to be contaminated it must be disposed of at <br /> an approved disposal location. Treatment of soil onsite can only be performed under a <br /> hazardous waste treatment permit. <br /> 2. A copy of the "Site Health & Safety Plan" must be on the jobsite for review. <br /> 3. See pages 7 & 8 of the permit application for required analytical tests and number of <br /> samples to be taken and witnessed by this department. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) All analytical results must come directly from a third party California certified <br /> laboratory with affixed chain of custody or this office will not accept them. <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form (DTSC farm 1249) <br /> (d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt <br /> 6. This permit expires on July 5, 2005. <br /> 7.
