Lindsay Street <br /> Stockton, CA 95205-4498 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the application for a permit to replace the defective line leak detector on the 87 grade gasoline <br /> underground storage tank (UST)with a new FE Petro Model STP-MLD leak detector on the <br /> underground UST system at the above noted address. <br /> The permit had been submitted to EHD retroactively after the leak detector was replaced and <br /> tested to be able to detect a 3-gallon per hour leak at 10 psi on June 30, 2010. The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. If concrete will be broken near the UST, a secondary containment test must be <br /> performed on the UST once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturers)of the component(s) <br /> being tested, repaired, or serviced. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br />
