Hermon Road, #206 <br /> Scotts Valley, California 95066 <br /> INJURY AND ILLNESS <br /> PREVENTION PROGRAM <br /> Tank Excavators ("the Company") is committed to providing a safe <br /> and healthful work place for our employees. The Company has created <br /> an Injury and Illness Prevention Program (the "Program") which is in <br /> accordance with Title 8 of the California Code of Regulations (CCR), <br /> Sections 3203 and 1509 of the General Industry Safety Orders, and <br /> Construction Industry Safety Orders, respectively. <br /> Components of the Illness and Injury Prevention Program include: <br /> management responsibility; safety communications; hazard assessment <br /> and control; procedures to investigate occupational Injury or illness, and <br /> corrective action procedures; health and safety training; and record <br /> keeping for the injury and illness prevention program. These components <br /> are described in further detail below. <br /> 1. MANAGEMENT AND ASSIGNMENT OF RESPONSIBILITIES <br /> Mr. Robert Smith has the authority and responsibility for imple- <br /> menting the Program. He will implement the Program to the best of his <br /> ability through utilization of the following Program elements. <br /> I I . SAFETY COMMUNICATIONS <br /> Tank Excavators' employs approximately 20 employees. Periodic <br /> meetings of employees are held when there is the need to discuss safety <br /> concerns of a general or specific nature. The Company will post written <br /> notice of the discussed safety concerns. <br /> -1- <br />
