IV. HEALTH AND SAFETY TRAINING <br /> Tank Excavators provides OSHA-approved health and safety training <br /> for employees when mandated by federal and/or state requirements. The <br /> initial 40-hour training course is updated annually with an 8-hour re- <br /> fresher course. <br /> Not all Tank Excavators' employees are required to have OSHA- <br /> approved training. These employees are trained in general safe work <br /> practices and as their specific jobs warrant. <br /> General safety training includes an overview of the employee's <br /> specific job duties and hazards associated with those duties, as well as <br /> the Company's Code of Safe Work Practices. See Attachment D, 'Employee <br /> Job Hazards'. <br /> V. RECORD KEEPING FOR THE INJURY AND ILLNESS PREVENTION PROGRAM <br /> Records maintained in support of the Company's Illness and Injury <br /> Prevention Program include: <br /> 1. Equipment and Work Practices Inspections <br /> These records include: the person conducting the inspection, <br /> the unsafe conditions/work practices found (if any), and the <br /> action taken to correct the conditions. (See Attachment A.) <br /> These records will be maintained for 3 years for those employees <br /> who have worked for the Company longer than one year. <br /> 2. Documentation of Health and Safety Training <br /> Employee health and safety training records include employee <br /> name, training dates, types of training, and training provider. <br /> Training records are maintained by Tank Excavators until termin- <br /> ation of the employee. <br /> 3. OSHA 200 Log of Occupational Injuries and Illnesses <br /> 4 - <br />
