Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by February 24, 2011. <br />7. The alarm log did not include a L2 alarm on October 27, 2010 and a L4 alarm on November 13, 2010. <br />These records shall be kept on site for at least 3 years. Immediately update the alarm log to include <br />these alarms. State what was done to correct these alarm conditions. Submit a copy of the updated <br />alarm log to the EHD by February 24, 204Q. Z�,L�d t <br />I <br />13. Bravo box float and chains in the under dispenser containments 1, 3 and 5 failed to stop the flow of <br />product at the dispenser when tested. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service <br />tephnician adjusted the chains, added missing cotter pins and verified functionality during the inspection. <br />17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br />of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, <br />retested it, and it passed. <br />�7 <br />22. Diesel observed in under dispenser containment 1 on January 24, 2011 was not recorded. Any <br />release from the primary containment that does not escape the secondary containment and was cleaned <br />up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. The inspector gave the operator a copy of the recordable release form.
