Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility by February 24, 2011. 7. The alarm log did not include a L2 alarm on October 27, 2010 and a L4 alarm on November 13, 2010. These records shall be kept on site for at least 3 years. Immediately update the alarm log to include these alarms. State what was done to correct these alarm conditions. Bravo box float and chains in the under dispenser containments 1, 3 and 5 failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service tephnician adjusted the chains, added missing cotter pins and verified functionality during the inspection. 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. �7 22. Diesel observed in under dispenser containment 1 on January 24, 2011 was not recorded. Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information: 1. Operator's name and phone number 2. List of types, quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring records. The inspector gave the operator a copy of the recordable release form. Submit a copy of the recordable release statement to the EHD by February 24, 2011. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
