CLASS II, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />104. Current financial responsibility documents have not been submitted to the EHD. The most recent <br />copy submitted to EHD was dated 3-28-13. Current financial responsibility documents are required to be <br />submitted annually. Immediately submit a copy of the financial responsibility documents to the EHD. <br />Alternatively, lag into CERS, upload the required information, and submit for review by the EHD. <br />Complete by 5-21-14. <br />201. The facility alarm log for the L5 91/diesel annular sensor fuel alarm on 4-18-13 was not on site <br />nor did the designated operator document this alarm. The secondary containment test result from <br />10-24-11 was not on site. These records shall be kept on site for at least 3 years. Immediately locate <br />and maintain the facility alarm log on site and submit copies to the EHD by May 21, 2014. Maintain a <br />copy of the facility UST secondary containment test results. <br />204. The designated operator failed to document the alarm from the attached alarm history on the <br />5-6-13 designated operator monthly inspection report and failed to document that it was responded to <br />appropriately. The missing alarm includes: L5 91/diesel annular sensor fuel alarm on 4-18-13. The <br />alarm history report was not attached to the 9-16-13 designated operator monthly report. During the <br />monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br />with documentation taken in response to any alarms to the monthly report. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports. <br />303. The diesel and 91 turbine sump sensors were touching the bottom of the sump but were located a <br />couple inches away from the lowest point, and not able to detect a leak at the earliest opportunity. <br />Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br />During the inspection, the technician repositioned the sensors to the lowest point of the sump to detect a <br />leak at the earliest opportunity.
