III Correction: (7-Eleven to make corrections) Item#9 Product was found in the Plus(Midgrade)STP sump and dispenser#05/06-Remove product and determine the cause of the release-provide a maintenance receipt-correct immediately. Correction 05/13/05 found Environ product line at(89)STP sump leaking at coupler-tightened coupler&sealed leak- removed Product from sump&placed in approved drum on site for 7-Eleven disposal at authorized site. Opened Dispenser#05/06&could find no liquid preset at this time-found#05/06(89)pining was shiny from old sheen,wined down pipe shear valve&pressurized line for aoorox 3 hours&observed no visible leaks or seeps at this time. 05/18/05 Performed follow-uw inspection& removed aoorox 1/4 gallon gasoline from(89)STP sumo(need seals on product line) Checked out Dispenser#05/06&found eel like residue on shear valve,wiped clean&observed for aoorox 30 minutes for seepage Shut-down#05/06(89)(may need new shear valve) 05/24/05 Checked out(89)STP sumo&product line&found no fuel or leaks in Plumbine at this time-(no liquid Present at this time) Removed shear valve top from#05/06(89)and resealed upper&lower union threads to nipples,reassembled &tested(ok)also itheed unions at#05/06(87)(91)&tested(ok)(no liquid present at this time) Item#11 The sensor in Dispenser#07/08 was not located on the bottom- Correction:Technician correct on site. Item#15 The secondary containment test results show failures from 01/08/04 for dispenser#05/06,#07/08 and the Regular east pipe line. These failures should have been repaired within 180 days of testing,provide documentation after test repairs and retest have been made-correct immediately. yyy Correction: (7-Eleven to follow-up on correction-See Marla Maples) pl Item#21 Repeat violation-liquid was found in the premium and regular fill sumps,all vapor&spill buckets-remove liquid and retain a receipt on site for proper disposal-correct immediately. Correction:05/13/05 Removed liquid from all fill snows,vapor&fill buckets&olaced in approved drum on site for 7- Eleven disposal at authorized site 05/18/05 Removed approx Y cup water from(89)fill bucket&placed in anwroved drum on site for 7-Eleven disposal at authorized site. Item#22 Provide a recordable release statement for the Plus(89)Midgrade STP sump-correct immediately.
