You requested that the monitoring plan be found or placed onsite. <br /> Resolution 1: The monitoring plan was placed onsite by our Designated Operator on 05/01/2018. <br /> Item 2: On your inspection report you noted that there was an employee that needed to receive their <br /> annual training from their Designated Operator. <br /> Resolution 2: The employee was trained by the Designated Operator on 05/01/2018 and the training <br /> log has been attached for your reference. <br /> Item 3: On your inspection report you noted that two manifests (009697876FLE & 00969640FLE) were <br /> not sent to the DTSC within 30 days of shipping waste. You requested that the manifests be sent to the <br /> DISC. <br /> Resolution 3: The manifests noted above were sent to the DTSC on 5/17/17 by our waste hauler, <br /> Belshire Environmental, Inc. Attached you will find their records indicating when the manifests in <br /> question were sent to the DISC. <br /> This letter should address the item(s) noted on the Inspection Report. 7-Eleven assumes that all issues <br /> regarding this report have been resolved and are closed at this time.
