SUMMARY OF VIOLATIONS GLASS I,CLASS II,or MINOR-Notice to Comply) On site to conduct a routine inspection and witness the annual monitoring system certification testing. 109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 5-23-13 and a test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to the EHD. 205. Nirmaljit Badial stated that he was hired in 2007. Vanessa Coss was hired approximately 13 months ago. The documentation for the employees trained by the designated operator does not indicate these employees' hire dates. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms A list of facility employees who have been trained by the designated UST operators(s) shall be maintained on-site or off-site at a readily available location, if approved by the local agency. The list shall be provided to the local agency upon request. The list shall include the dates of training for all facility employees, and the hiring dates for all facility employees hired on or after July 1, 2005. Ensure that employees have been trained by the designated operator, maintain the correctly completed list on site, a and submit a copy of the training records to the EHD by 6-13-14. 204. The designated operator failed to check and note the correct test dates for the monitoring system and spill container tests on the 4-15-14 designated operator monthly inspection report. These tests were listed as 5-23-14. These tests should have been listed as 5-23-13. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by June 13, 2014. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
