WAO <br /> HEVM <br /> 7-Eleven, Inc. <br /> Stephen Boyd <br /> Direct Dial Number(714)771-5484 <br /> Cellular(916)340-4182 <br /> Via Email Delivery <br /> June 25,2013 <br /> San Joaquin County Environmental Health Department RECEIVED <br /> 600 East Main Street Stockton, CA 95202-3029 <br /> Attn:Aris Cacapit JUN 2 0 42 0 ) <br /> Re: 7-Eleven Store No.: 32190 <br /> 4943 South Highway 99 SAN JOAQUIN COUNTY <br /> Stockton, CA ENVIRONMENTAL <br /> Inspection Report <br /> HEALTH DEPARTMENT <br /> Dear Mr. Cacapit: <br /> The intent of this letter is to respond to the items you noted on your Inspection Report issued to the <br /> location referenced above dated 5123/2013. <br /> Item 1: During your inspection you observed approximately one liter of fuel removed from the 87 turbine <br /> sump and asked that a recordable release statement be submitted and maintained on site. <br /> Resolution 1: A copy of the recordable release statement is attached and a copy was forwarded to the <br /> facility for their records. <br /> Item 2: On you inspection report you noted that the hire dates for several employees were not <br /> documented on the Designated Operator (DO) training form and asked that an updated training form be <br /> submitted. <br /> Resolution 2: It should be noted that not all store employees are forthcoming with their hire dates. I have <br /> asked our Designated Operators to obtain this information the as best they can. If the employee refuses to <br /> provide this information, I have asked the DO to record "not provided by employee"during future training <br /> events. A copy of the updated training log for this location is attached. <br /> Item 3: On your inspection report you noted that the test boots were left on and the test hoses were <br /> clamped off in the UDCs not allowing for communication from the piping secondary to the UDC. <br /> Resolution 3: It should be noted that there are no test boots in the UDCs when flexible piping is installed, <br /> they ones in question were penetration fittings. As for the test tubes and fitting plugs, from our <br /> conversation, this type of configuration was how equipment was traditionally installed as it would allow <br /> for the piping secondary to drain back to the turbine sumps similarly to ridged piping. Based on our <br /> conversation, I have requested all fitting plugs and testing tubes be removed from this location.
