I have <br /> asked our Designated Operators to obtain this information the as best they can. If the employee refuses to <br /> provide this information, I have asked the DO to record "not provided by employee"during future training <br /> events. A copy of the updated training log for this location is attached. <br /> Item 3: On your inspection report you noted that the test boots were left on and the test hoses were <br /> clamped off in the UDCs not allowing for communication from the piping secondary to the UDC. <br /> Resolution 3: It should be noted that there are no test boots in the UDCs when flexible piping is installed, <br /> they ones in question were penetration fittings. As for the test tubes and fitting plugs, from our <br /> conversation, this type of configuration was how equipment was traditionally installed as it would allow <br /> for the piping secondary to drain back to the turbine sumps similarly to ridged piping. Based on our <br /> conversation, I have requested all fitting plugs and testing tubes be removed from this location.
