Fill out the enclosed "Underground Storage Tank Unauthorized Release Report" <br /> form and submit within five (5) days if not already done. <br /> 2. Acquire the services of a qualified Engineering Consulting Firm (Hydrogeologist) <br /> to submit a plan for approval to this office for identifying the extent of <br /> contamination within thirty (30) days. <br /> 3. The Engineering Consulting firm (Hydrogeologist) shall submit a cleanup plan to <br /> this office for approval based upon the extent of contamination identified <br /> above. <br /> All necessary plan approvals and permits shall be obtained from the San Joaquin <br /> Local Health Districtrp for to starting any monitoring or cleanup operations. <br /> The existing unauthorized release of hazardous material at this facility is in <br /> violation of the California Health and Safety Code, California Water Code, and <br /> Titles 22 and 23 of the California Administrative Code. <br /> If you have any questions regarding this matter., contact the Environmental Health <br /> Division at (209) 466-6781. <br /> Jogi Khanna, M.D. , MPH <br /> District Healt ffi r <br /> C.
