EPA Site number. <br /> 2 . Sampling Firm and phone number. <br /> 3 . Clarification on which laboratory will perform soil <br /> analysis. 2 labs are listed. <br /> 4 . Hauler registration number for contaminated soil hazardous <br /> waste hauler. <br /> 5. Name and title of the site safety officer. <br /> 6. The destination and the hauler, including hauler <br /> registration number, EPA number, address and phone number, <br /> for waste oil/ spent photo developing solution. <br /> Submit the above items to PHS-EHD as soon as possible.
