The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was scheduled to be performed on November 19, 2014 and a test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 days of the test. Immediately submit a copy of the test report to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor. violation. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment test on the January 2015, December 2014 and November 2014 designated operator monthly inspection report. The test date on file is November 19, 2014, the date on the designated operator report was November 18, 2014. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. Overall Inspection Comments: This report was amended on February 23, 2015. Violation 106. CCR 2632 Monitoring and response plans is not current and/or not approved by the EHD was amended. The monitoring plan had the correct overfill as fill tube shutoff valve. Please change the overfill to fill tube shut off valve in the CERS submittal under the UST Tank Information/ Monitoring Plans for all of the fuel tanks and resubmit. The response plan was not uploaded into CERS during the time of inspection and therefore it is still a violation. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by February 29, 2015 . Consent to perform the inspection, take photos and collect samples was given by Charanjit Saini the store clerk. This facility's designated operator is Karen Amaiz (ICC expiration date: 5-31-15 ) .
