Pershing Ave, Stockton, CA 95207 SR0071731 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to remove the following underground storage tank (UST) systems, <br /> including piping and associated appurtenances, at the above noted address: <br /> Tank#TA0178506:•550 gallons used oil tank <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Removal of tank and piping. <br /> b. Verification that tank is cleaned to Title 22 CCR 67383.3(e)(2) standards. <br /> c. Witness soil and/or water sampling. <br /> d. Verification of Tank ID number TA0178506 marked on the UST. <br /> 4. If the removed backfill material and soil is found to be contaminated it must be disposed <br /> of at an approved disposal location. Treatment of soil onsite can only be performed <br /> under a hazardous waste treatment permit. A work plan and fees must be submitted to <br /> the EHD Site Mitigation Unit and the work plan must be approved before initiating any <br /> assessment or remediation activities. <br /> 5. Inert UST with 30 pounds of dry ice per 1,000 gallon volume after triple rinsing. <br /> 6. A copy of the submitted "Health and Safety Plan" must be on the jobsite for review. <br />
