The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aaron Hang at (209) 468-3453 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Since concrete will be broken over the 91-product tank, a secondary containment test must <br /> be performed on the 91-tank annular once concrete is poured and set in accordance with <br /> the manufacturer's guidelines. <br /> Please be aware that the facility cannot operate the UST system or receive fuel in the 91- <br /> product tank prior to the EHD inspection and approval. <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 1868 E.
