HOW TO USE THIS BOOKLET <br /> The ever sheet of this booklet contains useful information about the underground <br /> facilities at your station. Depending on the information given, you must use different <br /> forms in this booklet: <br /> 1. If your station has any single wall product tanks, use Form C. <br /> 2. If your station has any double wall product tanks, use Form B. <br /> 3. If your station has a single wall waste oil tank, use Form D. <br /> 4. If your station has a double wall waste oil tank, use Form B. <br /> 5. If your station has any double wall piping, use Form B. <br /> 6. If your station has a piping trench liner, use Form B. <br /> 7. If your station has an electronic monitoring system for any double wall piping or <br /> trench liner, you need not use Foran B for any double wall tanks or piping. <br /> B. If your station has vadose or groundwater monitoring wells, you still need to use <br /> Forms C and/or D as applicable. <br /> 9. If your station has other hazardous materials (see Daily Visual Monitoring, <br /> below) , you are responsible also for that portion of Form B. <br /> Also all stations mustlete Form A and send it in ev 3 months to the local a en <br /> shown on the cover sheet. <br /> In case of a leak or spill, you must complete Form E to attach to Form A, and you must send <br /> a copy of Form F to your Unocal Rep. within 24 hours. You must also notify your rep. by <br /> phone (and/or call the Unocal Emergency Phone after hours) . <br /> you must post a copy of Page 3 at a conspicuous location in your cashiers area. <br /> You must keep a copy of Form H to document the training received by your employees. <br /> ISP COPIES OF ALL FORMS YOU MAIL Ulf ! <br /> DAILY VISUAL MONITORING <br /> Hazardous Materials stored underground include: <br /> Gasoline <br /> Diesel Fuel <br /> Waste Oil <br /> These products are monitored for leaks in the underground tanks and piping. <br /> Hazardous Materials stored aboveground include: <br /> Propane <br /> Waste Oil (prior to dumping in underground tank) <br /> Motor Oil <br /> Transmission Oil <br /> Gear Lubricant (BOW/90) <br /> Grease <br /> Solvent (including parts cleaners) <br /> Battery Acid <br /> Antifreeze <br /> If you station stores any of these materials, the storage areas must be visually inspected <br /> every day for signs of leakage. <br /> If there is a leak or spill of any of the hazardous materials, whether stored above- or <br /> underground, you must follow the Emergency Response Procedures outlined on Page 3, as <br /> applicable. <br /> Page 4 of 10 <br />
